Eileen's care management story with Rhonda GuzmanOn January 31st, 2022, I met Eileen, the sharpest and most independent 98-year-old woman I had ever care-managed. Our journey together began at a rehab facility where she was recovering. Her deepest wish was to return to the home she’d lived in for 67 years, but unfortunately, it was no longer a safe option. Reluctantly, she agreed to move into an assisted living community, but the transition was tough. This is her care management story.

Eileen turned to me not only as her advocate but also as her friend and confidant. During our weekly visits, she shared her thoughts, worries, and most profound emotions. One of her biggest fears was dying alone without ever seeing her children again. Eileen’s family challenges were particularly complicated due to both distance and a somewhat estranged relationship with her children. My role expanded beyond care management, coaching her children on how to reconnect and support their mother from afar.

On June 18, 2023, Eileen reached an incredible milestone—her 100th birthday! As the day approached, it became clear that this would likely be her last. Her greatest wish was to have both her children together, something that hadn’t happened in 18 years. During my visits leading up to her birthday, I would say, “Take care of yourself! We have a big celebration coming!” Eileen listened.

On her special day, both of her children were there, and we celebrated her life in the most meaningful way.

Two days later, Eileen had a heart attack. Her daughter rushed to the hospital, sat by her side, held her hand, and put her brother on the phone. They were with her when she took her final breath.

Eileen knew her time was near. She didn’t want to die alone and was blessed to have her daughter by her side in those final moments. Ultimately, she was able to celebrate her life surrounded by her family and friends. Her last days were truly a “celebration of life,” just as she had wished.
