Phase 1: Noticing Changes: Early detection of dementia-related changes allows for more proactive management of the condition.

Phase 2: Increasing Dementia Symptoms: Your loved one is having difficulty communicating, experiencing challenging behaviors or refusing care. You need help figuring out how to approach, interact and elicit their participation successfully.

Phase 3: Needing Additional Resources: You are now worried about safety. You realize you cannot do this alone. You are concerned how to manage your own life and care of your loved one.  Additional resources are needed.

Phase 4: Finding A New Home: Your loved one is no longer able to live safely in their current home and they will need to move to a higher level of care. You need help identifying the best options.

Each individual’s experience with dementia or Alzheimer’s is unique.

Our process begins with a comprehensive assessment, during which we take the time to understand the specific challenges, preferences, and goals of the individual and their family. This includes evaluating cognitive abilities, physical health, behavioral patterns, and emotional well-being.

From this foundation, we develop a personalized care plan tailored to address medical, emotional, and social aspects of their life. This plan may include strategies to manage behaviors, enhance communication, and create a safe, engaging environment.

Whether it involves coordinating medical appointments, suggesting meaningful activities, or connecting families with additional resources, every detail is designed to improve the quality of life for both the individual and their caregivers.

This plan may include:

  • Cognitive Stimulation Activities: Engaging activities that promote mental function and preserve independence for as long as possible.

  • Safety Assessments: Evaluating the home environment to identify potential hazards and implementing solutions to ensure safety and comfort.

  • Medication Management: Coordinating with healthcare providers to manage medications effectively, minimizing side effects and ensuring adherence to treatment plans.

Emotional Support & Resources

The emotional impact of dementia and Alzheimer’s cannot be overstated. Families often face feelings of fear, frustration, and sadness. I provide guidance and resources to help families understand the disease process and cope with its challenges. This includes:

  • Education and Training: Helping families learn about the condition and effective communication strategies to enhance interactions with their loved ones.
  • Support Groups: Connecting families with local and virtual support groups to share experiences, gain insights, and find community.
  • Respite Care: Offering recommendations for respite services to provide family caregivers with necessary breaks, allowing them to recharge while ensuring their loved one receives quality care.

Advocacy & Coordination of Care

As an advocate for my clients, I work collaboratively with healthcare providers, legal advisors, and community resources to ensure that all aspects of care are coordinated effectively. This includes:

  • Care Transitions: Assisting families during transitions between care settings, such as from home to assisted living or skilled nursing facilities, ensuring continuity of care and minimizing disruption.
  • End-of-Life Planning: Providing guidance on advanced care planning, and facilitating discussions about values, wishes, and goals for care as individuals approach the end of life.
